Waiver Statement:

Release of Liability Waiver: On behalf of my child/children, I assume the risk and agree that Thomasville Center for the Arts and the staff shall not be liable in any way for any injuries sustained during classes and fieldtrips.
Health Acknowledgement:
1. I, as the parent/guardian, agree not to send my child/children to class if he or she is feeling sick, has an elevated temperature, or has been exposed to someone who is waiting for or tested positive for COVID-19. Being aware of my own child/children's health and physical condition, and having knowledge that his/her particiption may be injurous to his/her health, I am voluntarily giving permission for participation in after school classes at Thomasville Center for the Arts.
2. Having such knowledge, I hereby acknowledge this as a release for accidental illness which my child/children may incur as a result of participating in such activity. I hereby assume all risks connected and associated with said activity and consent to participation in said program.
